Note, forms for contributions holidays can be found at www.kiwisaver.govt.nz and should be sent to Inland Revenue or your employer.
Enrolment Form
Enrolment Forms are found at the back of the ASB KiwiSaver Scheme Investment Statement. Download Investment Statement and Enrolment Form. Note this file is around 2MB so it may take some time to download depending on your internet connection.

Current Prospectus
A copy of the current registered prospectus for the ASB KiwiSaver Scheme is available by emailing retire@asb.co.nz.
Switch Form
This form should be used if you wish to change how your ASB KiwiSaver Scheme savings are invested. You can choose to invest your existing and/or future ASB KiwiSaver Scheme savings into one or more other ASB KiwiSaver Scheme Investment Fund(s).
Lump Sum Contribution
This form should be used when you wish to make a lump sum contribution to your ASB KiwiSaver Scheme account.
Regular Contributions
To set up regular additional payments to the ASB KiwiSaver Scheme you should complete the Direct Debit form below. This authorises your bank to set up the payments from your bank account.
Withdrawal Request
Contact us on 0800 ASB RETIRE (0800 272 738) or retire@asb.co.nz for a form if you wish to apply to withdraw all or part of your ASB KiwiSaver Scheme savings, so we can provide you with the correct form for your circumstances.
Employer's Chosen Scheme Agreement (for employers only)
Use this form if you wish to choose ASB KiwiSaver Scheme as your Employer's Chosen Scheme for your business.
